To register to become an outpatient, please take the following steps:

Step 1: Make an Appointment

Step 2: Register on the Patient Portal & Complete the Forms

  • Create an Online Medical Records Profile

    It is our first preference that our patients use our ELECTRONIC MEDICAL RECORDS SYSTEM (CHARM PHR) to submit their medical history, questionnaires, and upload medical records, etc. This is the most secure method and we encourage our patients to participate in Electronic Health Records with Hawaii Naturopathic through CharmPHR. If you are able to do this great, if you feel challenged by technology or computers, we do not wish to add further stress for you, simply FILL IN THE FORM below and request that the forms be emailed to you or arrange to fill them in there at our office. Please complete this form and we will create an account for you.

  • Fill Out The Questionnaire

    Once your CharmPHR account has been created, we will send you the necessary forms to complete through the portal. If you are going to print and complete it, we will email the forms to you.

    We require that all forms be completed and any medical records gathered and submitted before your appointment.

    We’re located on 239 Haili Street on the corner of Kapiolani, right across from the Lyman Museum and the St. Joseph’s Catholic Church (big pink tower).
