Our Fecal Microbiota Transplantation (FMT) residential programs include fecal transplants for the treatment of Clostridium Difficile infection (C.Diff) and a comprehensive educational program for many other conditions including obesity, Crohn’s disease, ulcerative colitis, anxiety, depression and schizophrenia.
The fecal microbiota transplantation protocol features an organic plant-based raw and/or cooked diet, raw smoothies and juices, supplements and nutritional IVs, colon hydrotherapy and enemas.
The FMT procedure is administered in the building where you will be staying – the Aloha Wellness Center, downtown Hilo.
Food is served in Inn’s dining room and prepared fresh daily by our chef who will follow the dietary guidelines and instructions from the doctor. Meals are carefully planned to include ample resistant starch, prebiotic fibers and fermented foods to feed the good bacteria implanted from the donor.
You may rent a car on arrival at Hilo International Airport (ITO) or we can pick you up at the airport. We are a 10 minute drive from the Hilo airport. You may bring a family member for support in addition to the fecal donor if you wish. Your companion will be an integral part of the program, participate in the educational classes and may choose to follow a detox protocol supervised by our doctor.

Step 1: Start with a phone assessment
Call our office at 808-933-4400 and establish a portal of communication for you and your donor if you have already selected someone. Send your medical records including recent blood work. Fill out the Fecal Microbiota Transplantation (FMT) questionnaire and dietary questionnaire. Read the donor guidelines.
Step 2: Donor assessment
It is important to identify a healthy donor for a successful fecal transplant procedure. Your donor can be a family member or a friend. The donor will need to complete the following:
- required laboratory testing
- travel with you to participate in the program
- follow a specific diet prior to arrival and during the program
- be willing to toilet train if necessary to make his/her donation daily at a reliable time in the morning
Requirements of the donor:
- must have healthy stools similar to Type 2, 3 and 4 in the Bristol stool chart below (no constipation or diarrhea)
- needs to be free of chronic or acute illness
- may not have taken any antibiotics treatments in the prior 6 months
- may not have any inflammatory conditions due to food allergies or sensitivities
Step 3: Preparation – 2 weeks prior to arrival
- The donor has been selected and both of you will undergo a battery of blood and stool tests to rule out infections, evaluate your digestion/absorption status, and the quality of the intestinal flora.
- The donor’s diet exclude meat fish, dairy products, grains, soy, processed foods and drinks and include a wide variety of vegetables, fruits, seeds and nuts to exclusion of cashews and peanuts. If the patient has any particular food allergy or sensitivity the donor will exclude these from his her diet.
- The patient will follow specific dietary guidelines to starve identified pathogenic bacteria and encourage the growth of beneficial bacteria.
- Both patient and donor will keep a journal of their bowel movement consistency, color and timing.
Step 4: Residential 5 or 10 day program
The 5-day program for Fecal Microbiota Transplantation (FMT) includes two colonics, three infusions (transplants) of a fresh specimen and a chamomile enema. You may arrive on a Thursday afternoon and depart Monday afternoon.
Day 1: | Welcome, dinner and orientation. |

Day 2: | Juices for the patient and meals and juices for the donor. Donor collection of samples. Samples are frozen. Patient receives colonic #1 in the evening. Nutrient IV therapy may include: rehydration, Myers cocktail, Multi-mineral and vitamins, glutathione. |
Day 3: | Colonic #2 in the morning and FMT #1, smoothies and juices for the patient. |
Day 4: | FMT #2, continue with smoothies maybe solid foods (assessed for each individual), prebiotics. Nutritional coaching. |
Day 5: | FMT #3 continue with smoothies and solid foods, discharge instructions and guidelines to continue program/protocol at home. |
A 10-day program: | The 10 day program includes 9 fecal infusions/transplants, 2 nutritional coaching sessions and 2 nutritional IVs. |
